I hereby apply to participate in the St. Elizabeth Ann Seton Church CYO Basketball Program. I understand that participating in the Program will expose me to above normal risks of injury or harm. These risks include playing surfaces, physical contact or collisions with other players, spectators, or inanimate objects on or about the court. I understand that the sport of basketball is in itself hazardous and may result in injury to me or other players. 

I represent that I have no health or physical problems that will interfere with my participation in the Program. I agree that I am responsible for my own safety. 

I hereby assume all risks associated with my attendance and participation in the St.  Elizabeth Ann Seton Church CYO Basketball Program. I understand that I am solely responsible for any injuries which may occur to me as a result of my participation in the St. Elizabeth Ann Seton Church CYO Basketball Program. 

I understand that my playing time is not guaranteed, and that the minutes I receive are a reflection of my progress in practice, my place on a depth chart, and my coaches’ belief in my abilities in a given game situation.

I understand that my participation in Seton CYO also comes with the requirement of attending one (1) volunteer service project, to promote community service. 

By typing my name below, I agree to the waiver and release of liability conditions of this agreement, as well as the service requirement.  I will also print and sign a copy of the agreement found on the website. 

I sign this release on his behalf, and hereby agree to indemnify and hold harmless St. Elizabeth Ann Seton Church, their agents or representatives, from any loss, damage or expense sustained or incurred by them arising from any claim, cause of action or liability which may be brought by my minor child. 

I also understand that I will be asked to volunteer for 2 (two) home games, working either at the front desk collecting entrance fees, or behind the snack bar, or running the game clock, or keeping the scorebook.

By typing my name below as the parent/guardian of the child mentioned above, I agree to the waiver and release of liability conditions of this agreement, as well as the service requirement.  I will also be sure to print and sign a copy of the agreement found on the website.
