By checking the box below, I grant my child to participate in this parish event that requires transportation to a location away from St. Elizabeth Ann Seton. This activity will take place under the guidance and direction of St. Elizabeth Ann Seton employees and or volunteers.As a parent and/or gaurdian, I remain legally responsible for any personal actions taken by the above named minor ("Participant")
Parents/guardians of participants are advised that photographs or videotape of participants may be used in publications, websites or other materials produced from time to time by the parish. (Participants would not be identified, without specific written consent.). Parents/guardians who do not wish their child(ren) to be photographed or filmed should so notify the parish in writing. Please note the parish has no control over the use of photographs or film taken by private individuals or the media that may be covering the event in which your child(ren) participate(s).
I agree on behalf of myself, my child named herein, or our heirs, successors, and assigns, to hold harmless and defend St. Elizabeth Ann Seton Church.
Its officers, directors, employees and agents, and the Roman Catholic Diocese of Syracuse, its employees and agents, chaperons, or representatives associated with the event, from any claim arising from or in connection with my child attending the event or in connection with any illness or injury (including death) or cost of medical treatment in connection therewith, and I agree to compensate the parish/school, its officers, directors and agents, and the Roman Catholic Diocese of Syracuse, its employees and agents and chaperons, or representative associated with the event for reasonable attorney’s fees and expenses which may incur in any action brought against them as a result of such injury or damage, unless such claim arises from the negligence of the parish/school or the Roman Catholic Diocese of Syracuse.
